This information notice has been prepared by YKM SAĞLIK HİZMETLERİ LİMİTED ŞİRKETİ (İDEA AĞIZ VE DİŞ SAĞLIĞI POLİKLİNİĞİ) acting as the data controller (hereinafter referred to as YKM) within the scope of Article 10 of Law No. 6698 on the Protection of Personal Data and the Communiqué on the Procedures and Principles to be Followed in Fulfilling the Obligation to Inform, in order to fulfil the obligation to inform regarding the use of the contact form on our website.
The “personal data such as your name, surname, phone number, e-mail, etc., the messages you write in the form, and if you have uploaded them, your special category health information and files” that you share via the contact forms on our website are processed for the purposes of carrying out the necessary activities to enable you to benefit from the oral and dental health services offered by our company, conducting the relevant business processes, and contacting you regarding the matter you applied for and, if you have requested it, confirming your appointment.
The said data are processed by automated means, limited and proportionate to your purpose of submission, within the framework envisaged by the Law, based on the legal grounds set forth in Article 5 of the Law: “being mandatory for the data controller to fulfil its legal obligation” and “carrying out preventive medicine, medical diagnosis, treatment and care services”. They may be transferred to judicial authorities or the relevant law enforcement agencies in order to resolve legal disputes or upon request, as required by the relevant legislation.
In the event that you do NOT benefit from our oral and dental health services, your personal data will be stored for 1 (one) year in line with the purposes stated above. After the period expires, your personal data will be deleted, destroyed and/or anonymized by YKM in accordance with its policy on the processing, protection, storage and destruction of personal data, or upon your request, by the methods set forth under the Law on the Protection of Personal Data and the relevant regulations. Pursuant to the Law on the Protection of Personal Data, you are required to submit your request regarding the exercise of your rights stated above to our Company in writing. In order to exercise the rights stated above, you may fill in the YKM.F.15/04 personal data protection request application form, which is available on the Company’s website and can also be obtained from our patient rights unit (https://www.ideadentalclinic.com/kvkk), and submit it to our Company as indicated in the form, or you may also submit it to our Company via e-mail.
